Peoria Public Library - Passport Acceptance Facility

The Peoria Public Library Passport Acceptance Service processes only new applications, long-time (more than 5 years) expired applications and youth applications.

It is required that you please bring all older passport(s) to your appointment.

We do not process passport renewals at this time. For renewal information, please visit the State Department website at https://travel.state.gov

 

Tuesday, Wednesday, and Saturday service days all require appointments.
Please use the 'Schedule Appointment' button below to check availability and to schedule an appointment.
Passport Acceptance Services have limited walk-ins on Thursdays, from 3 p.m. to 6 p.m.
Passport photos are available on-site for $10.00 for passport clients.

 

Requirements For Passports

Complete all necessary forms.
Forms must be printed single sided and should be completed in black ink.

Provide proof of citizenship (original and a copy).
Certified birth certificate, passport, or naturalization certificate.

  • For minors 15 and under, child and both parents must be present.
  • For minors 16-17, child and one parent must be present.

Provide proof of identity (original and a copy).
Valid government issued picture ID.

  • For minors, parents must present valid ID.

A Passport photo.

  • Passport photos available on-site for $10.00 for passport clients.

Pay the U.S. Dept. of State by check or money order.
The U.S. Department of State passport fees must be paid by check or money order only (see fees posted below).

Pay the Peoria Public Library.

  • Execution Fee: $35.00 Must be paid by credit or debit card.
  • Photocopying Fee (if necessary): $0.20 per page.
  • Passport photos (if necessary): $10.00 for 2.

Tuesdays: 3 p.m. - 6 p.m. Appointment only.
Wednesdays: 3 p.m. - 6 p.m. Appointment only.
Thursdays: Limited walk-in appointments from 3 p.m. - 6 p.m.*
Saturdays: 9 a.m. - 12 p.m. & 1 p.m. - 4 p.m. Appointment only.

*Limited walk-in appointments are available on Thursdays from 3 p.m. - 6 p.m. The last appointment accepted must be present in the passport office no later than 6 p.m.

If you are 15 minutes late to your appointment or you do not have the appropriate documentation, you will need to reschedule the appointment.

Passport Fees

Payment #1: U.S. Dept. of State
Must be paid with money orders or checks (personal, certified, cashier’s, traveler’s) payable to “U.S. Department of State”.

  • Adult (16+) Passport Book: $130
  • Adult Passport Card: $30
  • Minor (Under 16) Passport Book: $100
  • Minor (Under 16) Passport Card: $15

Payment #2: Facility Fees
Must be paid in Debit or Credit. Only Visa and Master Card accepted.

  • Execution Fee: $35
  • Copy Fee: $0.20 per page.
  • Photo: $10.00 for 2 passport photos.

Processing Times & Expedited Fees

  • Routine Time: 4 to 6 weeks
  • Expedited Time: 2 to 3 weeks
     
  • Expedite Fee: $60 + Payments 1 & 2
  • 1-2 Day Delivery (book only): $21.36 + Exp. fee + Payments 1 & 2

Processing times begin the day the application is received at a passport agency or center, not the day the application was mailed or executed at an acceptance facility.
